Renewal is required every year. You do not need to fly out of the country to apply for the visa at an Indonesian Embassy or incur the Embassy visa application fee.
Renewal needs to be processed a minimum of 1 month before your Kitas Expired.

HOW TO APPLY FOR AND THE BENEFITS OF DEPENDENT KITAS
A Dependent KITAS (Temporary Stay Permit) in Indonesia is a type
of visa that is issued to foreign nationals who are the dependents
of a holder of a KITAS (Temporary Stay Permit) or KITAP
(Permanent Stay Permit).
This type of visa is issued to the spouse, children, or other
dependents of the KITAS/KITAP holder, and allows them to live in
Indonesia for the same period of time as the KITAS/KITAP holder
The Dependent KITAS holder is also eligible to work and study in
Indonesia, but is not allowed to engage in any business activities.
